Position:
Clinical Manager-RN
Position-M-F 8a-4:30p
Position Summary:
Reporting to the Director of Patient Care Services, the Clinical Manager oversees an interdisciplinary team of nurses, social workers, chaplains and hospice aides who provide end of life care in Long Term Care Facilities, Assisted Living Facilities and Private Homes. Responsibilities include, staffing, orientating new employees, clinical competency assessment, performance management and oversight of contracted services.
Qualifications:
· Professional Registered Nurse, currently licensed in Massachusetts.
· Bachelor’s degree in nursing
· Minimum of five years related experience in hospice, oncology, or home care.
· Minimum of three years management/supervisory experience.
· Demonstrated expertise in hospice philosophy and care, symptom management and assessment skills.
· Ability to supervise, guide and develop the skills of professional and non-professional staff.
· Ability to exercise independent judgement.
· Excellent verbal and written communications skills with individuals and groups.

Position:
Weekend Supervisor-RN
Baylor Position-Saturday and Sunday 8AM-8PM
Position Summary:
Under the direction of the Director of Inpatient and Home Care Services, the Weekend Supervisor performs functions to support the weekend operations related to the clinical services of Hospice of the North Shore’s (HNS) patients, families, physicians and clinical staff. The Weekend Supervisor is an integral member of the hospice team, working closely with interdisciplinary team (IDT) members across all patient care settings to coordinate care needs for hospice patients and families on weekends including planning, coordinating and supervising the activities of the weekend staff.
Qualifications:
· Current MA RN license in good standing.
· Minimum of three years of varied work experience as a professional nurse, preferably one of those years in an acute setting. Hospice nursing experience is highly desirable.
· Demonstrated ability to be self-directed, flexible and cooperative in fulfilling role obligation and ability to work effectively within an interdisciplinary team.
· Ability to supervise, guide and develop the skills of professional and non-professional staff
· Must have a valid MA driving license in good standing and reliable transportation as these roles require travel.
Position:
OnCall Nurse 7shifts on-7 nights off, 4:30pm - 8:0am (Full time, salaried, with full benefits)
Position Summary:
The On-Call Nurse works under the supervision of the Clinical Manager or designee during scheduled on-call hours. Responds promptly from home and with sensitivity to telephone inquiries from patients, family members, physicians, nursing homes and others regarding clinical issues. Utilizes clinical judgment to determine level of patient need, and make home visits as indicated and/or assigned. Schedule includes 7 days on and 7 days off from Wednesday to Wednesday, 4:30pm to 8:00 am on weekdays and 6:00pm to 8:00 am during the scheduled weekend, totaling 70 hours biweekly.
Qualifications:
· Current MA RN license in good standing.
· Minimum of three years of varied work experience as a professional nurse, preferably one of those years in an acute setting. Hospice nursing experience is highly desirable.

Position Summary:

We currently have openings in our Homecare division in which each will be responsible for traveling in our South Tier which will cover anywhere from Peabody/Saugus to Arlington/Lexington. We also have openings in our North Tier which cover areas from Danvers to Newburyport. Positions require occasional back up on call and holiday rotation.

Admissions / Liaison RN (2 positions)

Position Summary:
Reporting to the Admission Manager, the Admissions/Liaison Nurse is responsible for patient assessments and admissions to HNS, ensuring continuity of care between the admitting process and transition to ongoing care by the hospice interdisciplinary team (IDT). In addition, the Admissions/Liaison Nurse is responsible for educating health care professionals, patients, and families in all settings regarding the benefits/appropriateness of hospice care. The Admissions/Liaison Nurse is responsible for being proactive in identifying new referral sources, increasing referrals from existing sources, and providing educational programs to facilities. The Admissions/Liaison nurse maintains good communication with HNS clinical staff in order to enhance hospice services, and facilitates communication between HNS and referral sources.
Qualifications:
· Professional registered nurse, currently licensed in Massachusetts.
· Certified Adult Nurse Practitioner; certification in advanced practice palliative management preferred. Bachelor’s degree in nursing required;
· Master’s degree strongly preferred. Minimum of five (5) years nursing experience preferred.
· Minimum of two (2) years experience as a nurse practitioner preferred, ideally in an oncology, palliative care or hospice setting.
· One year additional work experience as a professional nurse in a homecare, hospital or skilled nursing facility with strong clinical assessment skills related to admission to or change in service.
· Must obtain and maintain certification for your position as defined in the HNS’s Operation Policy: Clinician Certification.
· Demonstrated understanding of Medicare/Medicaid benefit as it relates to all facilities settings.
· Experience in the use of electronic medical record preferred.
Position:
Weekend Baylor RN, Kaplan Family Hospice House
Sat/Sun 7:00AM-7:00PM
Position Summary:
The weekend Baylor RN will visit patients and provide assessment, direct patient care, and support to our hospice patients and families at the Kaplan Family Hospice House. Position will also admit patients as necessary. Work two 12 hours shifts, 7a-7p, every weekend. Work 24 hrs, get pay and benefits credit for 32 hours.
Qualifications:

Position Summary:
Under the clinical supervision of the Palliative Care Team Physician, the Palliative Care Nurse Practitioner (PCNP) provides consultation services as requested by area clinicians to address complex pain and symptom management issues for patients experiencing chronic and/or progressive life-limiting illness and their families. The PCNP participates in patient/family/provider education, as well as palliative care research and development opportunities for the Hospice of the North Shore. The PCNP engages in outreach activities to support the mission of Hospice of the North Shore (HNS).
Qualifications:
· Professional registered nurse, currently licensed in Massachusetts.
· Certified Adult Nurse Practitioner; certification in advanced practice palliative management preferred. Bachelor’s degree in nursing required;

Position Summary:
We are currently looking for hospice aides to provide care to patients in their homes, long term care and assisted living facilities in the evenings and on weekends.
· The Hospice Aide is responsible for providing and/or assisting the patient with personal care needs, ambulation, supplementary services including meal preparation, and other appropriate tasks as indicated by the Primary Nurse.
· Charting on the appropriate agency forms is also required.
Qualifications
· Minimum one year experience as either a Home Health Aide or CNA in home care or a long term care setting.
· Home Health Aide certification or CNA license in good standing with the nurse aide registry and not have had a lapse in a 24 month period.
· Must have a valid MA driver’s license in good standing with reliable transportation.
Position:
Hospice and Palliative Care Physician
1 weekend per month at Kaplan Family Hospice House
Position Summary:
Reporting to the Medical Director, the Hospice and Palliative Care Physician has responsibility for the direction of the medical component of assigned patients and is an integral member of the HNS interdisciplinary team (IDT). The Physician will also provide outreach and educational activities for HNS. Participates as a member of the Palliative Care Team in the development and implementation of the plan of care for each patient/family.
Qualifications:
· Current license to practice medicine in the Commonwealth of Massachusetts.
· Board certified with a minimum of two years experience in Internal Medicine, Family Practice or a related field.
· Privileges or eligible for privileges as an attending physician with one or more Hospice of the North Shore contracted acute care hospitals.
· Successful completion of a palliative care fellowship program strongly preferred. Eligibility and/or certification by the American Academy of Hospice and Palliative Medicine (AAHPM) desirable; willingness to obtain certification within three years required.
· Demonstrated competency in symptom control measures and appropriate palliative care for terminally ill patients.
· Demonstrated understanding of and support of an IDT approach to patient care planning and delivery.
Position:
Social Worker - LICSW (2 full-time positions, 1 part-time position)
Full Time South Homecare Team
Full Time LTC/ALF (Green Team)

Position Summary:
As a member of the interdisciplinary team, the social worker reports to a Clinical Manager, and is responsible for planning and providing supportive services to terminally ill patients and families. The social worker is responsible for identifying patient/family psychosocial needs, and for addressing those needs through direct service and/or consultation with other staff members in accordance with the hospice plan of care.
Qualifications:
· Master's degree in social work or related field preferred. Current Massachusetts social work licensure (LICSW preferred, LCSW accepted with appropriate work experience).
· Minimum three years counseling experience. Experience counseling terminally ill persons and their families; knowledge of the health care delivery system and community resources.
· Experience and training in leading support groups, and skill in the area of family psychosocial assessment.
· Demonstrated ability to assess and respond to the needs of patients and families in varied settings.
· Demonstrated understanding of hospice philosophy and principles.
· Demonstrated ability to be self-directed, flexible and cooperative in fulfilling role obligation, and ability to work effectively within an interdisciplinary team.
· Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and in good working order.

Medical Biller
Full-Time Monday through Friday
Position Summary:
Reporting to the Controller, the Medical Biller is responsible for all aspects of Long Term Care medical billing and collections
· Registers patients in the billing system, entering all pertinent billing and related information necessary for the accurate and timely submission of claims.
· Prepares and submits claims to third party insurance carriers either electronically or manually
· Coordinates third party insurance carriers' requests for medical documentation
· Follows up with third party insurance carriers on unpaid claims
· Follows up with individuals for unpaid self-pay balances
· Processes rejections by either balance billing accounts to private pay and generating a letter of rejection to patient or corrects any billing error and resubmitting claims to third party payers.
· Coordinates and maintains billing files and database, documentation, and correspondence related to the billing and collection accounts
· Performs other duties as assigned
Qualifications:
· Associate's degree, preferably in business
· Minimum of two years of third party medical billing and collection experience required
· Experience with Medicare Part A and/or LTC billing is required

Position Summary:
The Administrative Assistant is responsible for supporting Hospice of the North Shore’s (HNS) physician services and palliative care program related to services rendered to patients and families. Provides structure and daily administrative support, and helps to coordinate physician and advanced practice clinician’s activities and schedules to maximize patient services, productivity, and volume.
· Coordinates daily operational activities and scheduling of the HNS physicians and advanced practice clinicians according to patient need, referral source request, and clinician availability.
· Facilitates communication and notification between HNS physicians and advanced practice clinicians and attending physicians including notification of interventions and home visits.
· Coordinates obtaining medical records from hospitals,and physician offices as needed for HNS physicians and advanced practice clinicians.
· Coordinates the clinical activities of the HNS physicans by responding to requests for physician home visits for pain and symptom management issues and certification and re-certification processes.
· Provides administrative support to HNS Chief Medical Officer including calendar management, preparing letters, reports, presentations, forms, and data analysis as requested.
Qualifications:
· Associate’s degree or equivalent in business with health care related focus, Bachelor’s degree preferred.
· Three to five years of related experience preferably in a medical office administrative role including direct communication/intake by phone or in person with patients or patient families, working knowledge of medical terminology, and medical billing.
· Proven customer service skills dealing with a health care service provider.
· Ability to exercise independent judgment including the handling of highly confidential medical information.
· Intermediate level PC-based computer skills with proficiency in MS Office/Excel/Powerpoint software required.

Position Summary:
Reporting to the Triage Nurse and Patient Care Manager, the Clinical Team Assistant is responsible for providing clerical, secretarial and administrative support to the Patient Care Managers and members of the clinical team.
· Assist in the collection and processing daysheets.
· Assist in the maintenance of records and files

Reporting to the Vice President/Chief Operating Officer, the Director of Home Care and Volunteer Services is responsible for planning, implementing and directing all aspects of clinical services to assure the efficient delivery of quality hospice services to Hospice of the North Shore (HNS) patients in the home care setting and contracted inpatient facilities. Also provides oversight to ensure growth and management of all aspects of the HNS volunteer and bereavement programs, including strategic planning to promote opportunities and increased community participation.
Qualifications:
· Professional Registered Nurse currently licensed in Massachusetts.
· Bachelor’s degree required; Master’s degree in nursing, health care administration or related field preferred.
· Eight to ten years relevant experience, including clinical experience in hospice, home health, or long term care, including at least three years in a management role.
· Experience with staffing, performance evaluation, staff development, budget development and management, and performance/service improvement in a multi-discipline clinical setting.
· Experience with health care accreditation, regulations and licensing such as DPH, JCHAO, or CHAP.
